F/A-18 training-flight crash near the Susten Pass
A Swiss Air Force F/A-18C crashed during a training mission, killing its pilot. The reference material states that an incorrect altitude instruction from the Skyguide controller at Meiringen was assumed to be a cause.

Überlingen mid-air collision
A Bashkirian Airlines Tu-154 and a DHL Boeing 757 collided in southern Germany in airspace controlled by Skyguide. The accident killed 71 people and led to criminal proceedings involving four Skyguide employees.
