Belvidere, Illinois pressure-vessel explosion
A pressure vessel at NDK's Belvidere plant ruptured after reported stress-corrosion cracking and inadequate inspection or maintenance. Debris killed a nearby truck driver, injured another person, and damaged neighboring property. The incident was investigated by the U.S. Chemical Safety Board and later led to an OSHA penalty.
