Engine-room fire caused by fuel-line repair failure
A loose bolt in an improperly repaired fuel-line flange assembly allowed fuel to leak and ignite in the engine room. The fire was contained using the ship's emergency systems, but the vessel required repairs and incurred reported expenses and lost revenue exceeding $8.8 million.
