Investigation of febrile convulsions after H1N1 vaccination
Australian authorities investigated febrile convulsions reported in children after administration of CSL's Panvax and Panvax Junior pandemic H1N1 vaccines. Scientific publications proposed that a manufacturing-process factor might have contributed, although the event is best described as a regulatory and vaccine-safety investigation rather than a finding of intentional misconduct.

Bundaberg diphtheria tragedy
A diphtheria toxin-antitoxin batch associated with CSL was linked to the deaths of twelve children in Bundaberg, Queensland. Investigations did not find that the manufacturing process caused the deaths, but identified labeling failures. The incident became a major historical lesson in biological-product safety and quality control.
